Dr. Piro’s Liposuction techniques

Fluid Injection, a technique in which a medicated solution is injected into fatty areas before the fat is removed, is commonly used by plastic surgeons today. The fluid — a mixture of intravenous salt solution, lidocaine (a local anesthetic) and epinephrine (a drug that contracts blood vessels) — helps the fat be removed more easily, reduces blood loss and provides anesthesia during and after surgery. Fluid injection also helps to reduce the amount of bruising after surgery.

Large volumes of fluid — sometimes as much as three times the amount of fat to be removed — are injected in the tumescent technique. Tumescent liposuction, typically performed on patients who need only a local anesthetic, usually takes significantly longer than traditional liposuction (sometimes as long as 4 to 5 hours). However, because the injected fluid contains an adequate amount of anesthetic, additional anesthesia may not be necessary.

The super-wet technique is similar to the tumescent technique, except that lesser amounts of fluid are used. Usually the amount of fluid injected is equal to the amount of fat to be removed. This technique often requires IV sedation or general anesthesia and typically takes one to two hours of surgery time.

Ultrasound-Assisted Lipoplasty (UAL). This technique requires the use of a special cannula that produces ultrasonic energy. As it passes through the areas of fat, the energy explodes the walls of the fat cells, liquefying the fat. The fat is then removed with the traditional liposuction technique.

Facelift Information

The neck lift procedure is designed for people who have excess fat around the neck, but have good skin tone and are not yet ready for a full face-neck lift. This operation includes liposuction of the neck, manual removal of fat underneath the muscles and tightening of the neck muscles under the chin. Excess skin cannot be removed with this procedure. If excess skin is the problem then a full face-neck lift should be done.

A one inch incision is placed just under the chin, and the skin is elevated off of the fat and muscle. A 3mm cushion of fat is left on the skin, and then liposuction is used to remove all of the remaining fat down to the muscle. De-fatting the neck is also done by manually removing fat from underneath the platysma muscle. Once the fat is removed, the muscles are tightened and any neck cords are released.

The incision is then closed with 3-4 sutures. Drains are also used to remove any excess fluid or blood and are removed in 1 or 2 days after the surgery.

Are There Different Liposuction Techniques?

Though the basics of liposuction remain the same, there are different techniques that can be used during liposuction. "Tumescent Liposuction" During this technique the plastic surgeon will inject a medicated solution into the fatty areas before the fat is removed. The solution is a mixture of intravenous salt water, a local anesthetic (lidocaine) and a drug that contracts the blood vessels (epinephrine). This fluid injection helps the fat to be removed more easily, reduces blood loss and bruising after surgery, and provides anesthesia during and after the surgery. The amount of fluid injected varies and depends on the preference of the surgeon. "Ultrasound-Assisted Lipoplasty (UAL) This technique requires the use of a special cannula that produces ultrasonic energy. As the ultrasonic energy passes through the areas of fat it causes the fat cells to liquefy. The fat is then removed with the traditional liposuction technique. Because the fat is liquid at the time that it is removed, UAL is smoother and mechanically more efficient. Remove Localized Fat Deposits with Liposuction

Liposuction can remove localized fat deposits from almost any area of the body. Many plastic surgeons use an "ultrasonic" technique, during which sound waves liquefy fat cells before liposuction begins. Liposuction: The Procedure

Many plastic surgeons use a "tumescent technique." After the induction of general anesthesia or intravenous sedation, a large volume, up to several quarts, of very dilute local anesthetic is injected into the areas to be treated. There is a tiny dose of adrenaline in the fluid as well, which makes the blood vessels in the area clamp down. This prevents perhaps 95% of the blood loss that would otherwise occur during liposuction.
Thin hollow stainless steel tubes are placed through short (perhaps 1/4") incisions in the area treated and are used to suction away the appropriate amount of fat. Sheets of lightweight foam are placed on the treated areas, and a compression garment, like a corset, is placed. Bruising is usually scant.
